I - HOW TO APPLY?
- Accomplish the Water Permit Application Form in 4 copies

Prepare a Notice of Water Permit Application (WPA) in 10 copies and post them for 30 days at the following:
- Bulletin Board of NWRB
- Barangay Hall where project is located
- Municipal Hall where project is covered
- Provincial Capitol where project is covered
- District Engineer of the DPWH
- Regional Director’s Office of DPWH
- Regional Director’s Office of DENR
- Provincial Irrigation Office (NIA)
- Head office of NPC
- Water District (if existing)
- File the application and all necessary documents (refer to Section II below) at the Office of the National Water Resources Board
- Pay the required fees (see Section III below)
- Obtain first a Certificate of Water Availability
- Obtain the Conditional Water Permit (CWP)
- Comply and submit all remaining requirements
- Obtain the Water Permit
- Renew application at indicated date
- Visit http://www.nwrb.gov.ph/ for more information and guidance
II - WHAT ARE THE REQUIREMENTS?

Water Permit Application (For Power Use):
- 4 copies of duly accomplished and notarized Water Permit Application Form
- 10 copies of Notice of Water Permit Application

2 copies of Proof of land ownership on the site where the structures will be constructed, or any of the following:
- Certificate of Title;
- Tax Declaration;
- Lease Agreement;
- Deed of Donation;
- Certificate from Local Government Unit (LGU) to utilize the source

Location plan and vicinity map of water source (use scale of 1:50,000) showing:
- Point of diversion of source of water (Longitude and Latitude)
- Nature of diversion works whether temporary or permanent

SEC Certificate of Registration
- Corporation/Partnership from SEC with Articles of Incorporation and By-Laws, Certificate of Latest Corporate Financial Structure issued by the Corporate Secretary, and the list of incorporators;
- Cooperative from CDA;
- Single Proprietor from DTI;
- Water District from LWUA;
- Barangay Waterworks Association from LGU or equivalent regulatory agency
- Tax Identification Number (TIN) from BIR
- Environmental Compliance Certificate or Certificate of Exemption from DENR-EMB

Brief description of project which includes:
- How water will be used
- Amount of water needed
- Power expected to be generated
- Amount of water to be discharged back to source
- Measures to be taken to avoid water pollution
- Scheme of development
- Well drilling data (Pumping Test, Well Log Data, and Water Analysis) in case of existing groundwater source. Well driller must have a valid NWRB Certificate of Registration

Other requirements as deemed necessary by the board, such as but not limited to:
- Certificate of Endorsement from Department of Energy
- Certificate of Registration

Transfer of Water Permit:
- Petition for transfer
- Copy of Water Permit
- Posting Notice from DENR, Municipal Hall

With Conditional Water Permit
- Copy of the plans and specifications for the diversion works;
- Pump structure;
- Water measuring device and other required structures; and
- Daily stream flow data measurement of the proposed water source.
